We are looking for an experienced and dedicated Aboriginal Case Manager to join our team at Momentum Collective. As the Aboriginal Case Manager for Homelessness Services, you will play a vital role in supporting and advocating for individuals within the community who are experiencing homelessness or at risk of homelessness.
**Responsibilities**:
- Provide culturally appropriate case management and support services to First Nations people and their and families experiencing homelessness.
- Develop strong, trusting relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
- Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
- Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
- Conduct comprehensive assessments of clients' needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
- Maintain accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines.
- Participate in regular supervision sessions, team meetings, training programs, and professional development activities to enhance skills and knowledge relevant to the role.
**This is a full time fixed term role to 30th June 2026 with the possibility of on-going work. Thid position can be based in either our Tweed or Ballina offices.**

**ABOUT YOU**
- Cert IV Community Service and or other relevant qualification and or case work experience in an associated field
- Current Driver’s Licence
- Satisfactory Criminal record check.
- Current Working with Children Check (NSW)
- Understanding and experience of working with Aboriginal people and their communities.
- Demonstrated experience in case planning and coordination of people with complex support needs.
- Understanding of, and demonstrated ability to work with Human Service Agencies.
- Demonstrated experience and or understanding of Trauma Informed Care.
- Excellent communication skills, particularly in the areas of negotiation, facilitation, mediation and problem solving.
- Intermediate computer literacy skills including Microsoft office suite and databases.
