Social Worker (Broadmeadows)

**The Role** We are currently seeking a **Social Worker** to join our agile and growing team. In this role, you will work directly with participants to support their individual needs, goals, and aspirations, while promoting independence and community participation.

**Key Responsibilities**
- Provide person-centred and strengths-based support to NDIS participants.
- Assist participants with goal setting, skill development, and social inclusion.
- Facilitate access to community resources and services.
- Maintain accurate documentation and progress reports in line with NDIS requirements.
- Assist them to achieve their housing needs & goal
- Work collaboratively with families, carers, and other support professionals.

**About You** We’re looking for someone who is passionate, empathetic, and committed to making a positive difference in people’s lives.

**Essential Criteria**:

- Qualification in Social Work (Bachelor or Master of Social Work).
- Eligibility for AASW membership.
- Understanding of NDIS framework and disability sector.
- Excellent communication, organisational, and problem-solving skills.
- NDIS Worker Screening Check.

**Desirable**:

- Previous experience working with people with disability, mental health, or community services.
- Ability to work flexibly and adapt to diverse participant needs.

- $84.95 per hour
