Description
This is a fixed term, full time position commencing July 2026 up until December 2026. The Role As a Transition Officer you will provide individualised case coordination and support services to 10 to 18 year old Aboriginal students transitioning into the boarding environment. You will also provide support and advice to school staff, Aboriginal students and their families for students requiring transition and brokers access to other providers offering the most relevant service to each individual’s circumstances. This role includes actively participating in multidisciplinary team meetings and with other agencies as appropriate, providing information and consultancy advice as required. Responsibilities include:
- Researching, developing, implementing and monitoring high quality plans for young people to access appropriate pathways to recreational, training and employment options.
- Providing consultative support to colleagues.
- Maintaining a record system, analysing data and preparing information to develop and monitor plans.
- Developing and maintaining an effective network of youth, employment, education and training services relevant to young Aboriginal people.
- Connecting young Aboriginal people to appropriate local youth and counselling services. Professional Benefits

Eligibility
- Be an Australian or New Zealand citizen, permanent resident or have a valid visa with relevant work rights for the term of the appointment.
- Consent to a Nationally Coordinated Criminal History Check and obtain a current Screening Clearance Number issued by the Department of Education’s Screening Unit before you start.
- Obtain or hold a current Working with Children Check.
- Hold or obtain a current valid ‘C’ or ‘C-A’ class Western Australian drivers licence.
