About the Role The Patient Navigator is a key member of the administrative team, working closely with clinicians to support patients throughout their care pathway. This position requires managing patient flow, coordinating appointments, and facilitating communication between patients and the multidisciplinary care team. Key Responsibilities
- Act as the first point of contact for patients, carers and community referrers, responding to questions, resolving appointment issues, and clarifying instructions.
- Coordinate communication between patients and their clinical care team, ensuring timely and explicit information flow.
- Facilitate patient access by managing patient flow and assisting navigation through the cancer care system.
- Develop specialist knowledge across multiple tumour streams and build strong relationships with patients and multidisciplinary care teams.
- Ensure patients’ diagnostic, treatment and clinic review appointments are appropriately timed, sequenced and coordinated, including same‑day booking and telehealth alternatives when appropriate.
- Support tumour stream based clinics and Multidisciplinary Meetings by preparing required patient data and ensuring agendas run smoothly.
- Work closely with the Registrations and Referrals team to identify and prioritise urgent patients for registration and first appointments.
- Provide administrative support for weekly MDMs, including preparation of patient diagnostic and pre‑treatment assessment data and recording outcomes.
- Assist with the development and implementation of SOPs, quality improvement projects, and staff training where required.
- Maintain and update patient records in accordance with hospital policy and the Data Accountability Framework. Qualifications
- Relevant Certificate or Diploma in a health or administrative discipline, or several years of experience in an administration setting.
- Strong customer service skills with the ability to communicate effectively and respectfully with patients and staff.
- Proficiency with IT systems, preferably in a health care environment, and competence in the Microsoft Office Suite.
- Experience working in a healthcare, administrative or customer service role (healthcare experience highly regarded).
- Proactive and cooperative approach to problem‑solving.
- Desirable: experience providing direct line supervision, developing SOPs, participating in quality improvement projects, and using patient administration systems such as iPM and Epic. Remuneration Under the Health and Allied Services Enterprise Agreement 2021‑2025, salary ranges from HS Grade 2 Year 1 (HS2) $66,768.00 per annum to HS Grade 2 Year 5 (HS21) $73,811.92 per annum, depending on experience.
