Description
Call On Clare has been supporting older Australians to live independently in their own homes for many years. Due to ongoing growth, we’re looking for a compassionate and motivated Case Manager to join our team. In this role, you’ll help clients maintain their independence by providing expert case management and support coordination under the New Support at Home Program. You’ll work closely with clients and their families to develop personalised care plans that meet their needs, goals, and lifestyle. What we will offer you:

- Deliver case management and care coordination for Home Care Package/Support at Home Clients
- Build strong relationships with clients, families, and service providers
- Conduct assessments, referrals, and regular reviews
- Client intake and Care Consults
- Develop and manage individualised care plans and budgets
- Deliver excellent telephone-based client care for elderly Australians including Care Planning, Assessment and Coordination
- Use your customer service and aged care management skills to make a mark on the community
- Develop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionals
- Work in collaboration with our Nurses and other Managers to facilitate, develop and shape our service offerings
- Monitor the budget of care packages and ensure clients understand service fees
- Engage with internal teams to facilitate client on-boarding
- Ensure compliance with all relevant legislative and industry standards About You You’re a caring, genuine, and proactive individual with a passion for aged care and supporting older people to live well at home. You are confident working independently and have a strong ability to manage and prioritise a caseload. You will bring:
- Minimum 2 years’ experience in a similar Care Partner/ Case Manager role in Support at Home or Home Care Packages
- Background in nursing (EN/RN) or allied health preferred
- Use of Sandwai highly preferred
- Diploma in Community Services or Case Management, or willingness to commence studies
- Exceptional interpersonal, negotiation, and time management skills
- Robust IT skills and understanding of aged care quality standards
- Must be able to multitask
- Be willing to travel around for care consults Requirements:
- Current Victorian Driver’s Licence + own insured vehicle
- Current Police Check (within the last 12 months)
