Social Worker / Case Manager (Hawthorn East)

We are seeking a compassionate and motivated Social Worker / Case Manager to join our growing team. The successful candidate will support individuals and families through advocacy, case management, counselling support, and coordination of community services. The role is ideal for someone with experience in healthcare, community services, mental health, rehabilitation, or correctional environments. Key Responsibilities
- Provide case management and client support services
- Conduct client assessments and develop care plans
- Liaise with hospitals, community organisations, government agencies, and support providers
- Support vulnerable individuals experiencing social, emotional, behavioural, or mental health challenges
- Prepare reports, case notes, referrals, and documentation
- Assist clients with housing, rehabilitation, NDIS, employment, and community integration
- Work collaboratively within multidisciplinary teams
- Maintain confidentiality and professional standards at all times Requirements
- Qualification in Social Work, Community Services, Psychology, Counselling, or related field
- Previous experience in community services, healthcare, mental health, or social support roles preferred
- Experience working within correctional centres, rehabilitation services, youth justice, or custodial environments highly regarded
- Strong communication and interpersonal skills
- Ability to manage challenging situations professionally
- Knowledge of trauma-informed care and case management principles
- Current Working With Children Check and Police Check preferred
