- Mount Alexander Reference: VG/ Dhelkaya Health is seeking a motivated and organised Post‑Acute Care (PAC) Coordinator to join our Community Nursing team. This role supports people living in the Mount Alexander, Macedon Ranges and Central Goldfields shires following discharge from hospital. Reporting to the Community Nursing Unit Manager, the PAC Coordinator plays a vital role in delivering person‑centred, community‑based care during the first month after hospital discharge, helping people to recover safely and regain independence at home. The Post‑Acute Care Program coordinates short‑term community services to support recovery following a hospital admission. As PAC Coordinator, you will:
- Receive, assess and process post‑acute care referrals in a timely manner
- Apply clinical knowledge to develop individualised, person‑centred packages of care
- Coordinate and broker community‑based services including district nursing, allied health, occupational therapy, physiotherapy, and personal, domestic and home care supports
- Manage fluctuating demand while maintaining efficient workflows
- Support transition planning and discharge from the PAC program
- Work closely with administrative staff to ensure accurate processing and reconciliation of service delivery About you You are an experienced and organised health qualified who can work independently while collaborating effectively with a wide range of stakeholders. You will bring:
