About the Team CHQ at Home provides a range of in‑the‑home health care services for children and young people living in the broader Brisbane metropolitan area. It includes Hospital in the Home (HITH), Post Acute Care (PAC), Hospital Avoidance (HA) and Palliative Care Programs. HITH provides care in the home workplace as an alternative to hospital inpatient stay for infants and children whose condition is reliable or predictable. Patients can be transferred from an inpatient ward, shortening their length of stay in hospital, or admitted directly to HITH from an outpatient clinic or emergency department, avoiding an inpatient hospital stay altogether. Responsibilities Actively participate in a working workplace supporting quality human resources management practices including employment equity, anti-discrimination, work, health and safety, and ethical behaviour. Develop a positive culture within your division which encourages and recognises high performance, builds leadership capabilities, and supports staff to maximise their health and wellbeing.
