**About the Role**: **About Us**: The Monash Health Community Rehabilitation team is looking for a Grade 2 Social Worker to join our Kingston and Cranbourne home based teams on a temporary basis (annual leave cover). The role presents an excellent opportunity to broaden your skills and gain experience in the Community setting, and will be well suited to an experienced Grade 1 or a Grade 2 Social Worker looking for a change.

This position is responsible for the care of adult clients in Community Rehabilitation across a variety of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation. The team is well supported with a Senior Clinician Social Worker and Team Leaders.

- Bachelor of Social Work OR Masters of Social Work Qualifying OR Master of Social Work OR equivalent.
- Relevant professional experience working with adults in a rehabilitation, community or case management setting
