Job responsibilities: Document assessments, interventions, and patient progress in the medical record. Provide patient and family education related to the plan of care and discharge needs. Support secure transitions of care, including handoffs and care plan updates. Work 36 hours per week with 12-hour shifts. Coordinate patient care activities with physicians and other clinical team members.
