Position: Community Palliative Care Nurse Location: Numurkah EFT/Hours: 0.8 EFT (64 Hours Per Fortnight) Status: Fixed Term Part Time (3 months) Hourly Rate: $ per hour plus 12% superannuation About the Role NCN Health have an exciting opportunity for a Community Palliative Care Nurse to join our team in Palliative Care at Numurkah. The Community Palliative Care Nurse is responsible for supporting individuals with life limiting illness and their carers/family to optimise quality of life across the continuum of palliative care, both the home environment and other care settings across the Moira Shire catchment area. The position will provide and coordinate supportive care to meet the complex clinical, social, emotional and spiritual needs of individuals with life limiting illness, through high level clinical and psychosocial assessment, planning, implementation and evaluation of palliative care service provision in partnership with General Practitioners, specialist consultants, District Nursing Services, Allied Health Professionals and other health and community-based services. The Community Palliative Care Nurse also plays a pivotal role in providing advice, support and education to the family and carers and of people referred for support, and may at times be required to provide on-call support after-hours to ensure clients and their family are supported during end-of-life care. Under the governance of NCN Health, the Moira Palliative Care Service provides community-based palliative care services across the Moira Shire catchment area. About You
- Vaccination against seasonal Influenza (mandatory)
- Vaccination against Covid-19 (recommended)
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Post graduate qualifications in palliative care or working towards same.
- Minimum of two years clinical nursing experience in palliative care or related area.
- Competent in using Microsoft products (Outlook, Word, Excel) and well-developed computer skills, including the use of tablets/hand held devices at point of care.
