About the role: As a Post-Custodial Support Case Manager with us, you will get to support people as they transition from custody back into the community, helping them build stability, wellbeing, and a positive future. You’ll work alongside participants before and after release, providing practical and therapeutic support to reduce harmful AOD use, prevent overdose and suicide, and lower the risk of re‑offending. - Support individual journeys through assessment, planning, and goal‑focused counselling
- Connect people with AOD, health, housing, employment, and training supports
- Strengthen family and community connections
- Build everyday living skills that promote independence and confidence Hours and location: The hours are 22.5 per week and days are flexible. This position is based in Coffs Harbour with travel to Grafton. The role is hybrid with options to work from an office space, home, in correctional centres and in the community. A vehicle is available from a central location. The role is funded until 30th September 2027 with potential for extension. About you: Selection criteria Qualifications and checks
- Diploma level qualification or above in Community Services, Social Work, Mental Health, Justice, Psychology and/or Welfare Studies
- Current driver's licence, a satisfactory Criminal Record Check, Working with Children Check and ability to obtain clearance to attend correctional facilities and undergo required vaccinations Professional skills and experience
- Experience working within AOD, criminal justice or another relevant sector with an understanding of complex clinical presentations
- Substantial case work experience to support a range of practical and psychosocial needs and community support for people who have experienced incarceration.
