**Achieve Something Great** The Transitional Aged Care Program (TACP) offers a rewarding opportunity to make a genuine difference in the lives of older adults by supporting reablement, independence, and the achievement of personal goals.As a Dietitian within the Residential TACP, you will play a vital role in delivering high‑quality nutrition assessment and intervention to older adults undergoing short‑term restorative care in a residential aged care facility. This unique program supports clients following hospital discharge, enabling them to build independence and confidence prior to transitioning back home in the community.
In this role, you will:
- Undertake comprehensive dietetic assessments and develop individualised, evidence‑based nutrition care plans
- Provide targeted nutrition interventions to support malnutrition management, frailty, wound healing, and chronic disease management
- Deliver nutrition education to clients and families, supporting informed decision‑making and goal achievement
- Work collaboratively within a multidisciplinary team to optimise client outcomes
- Contribute to a client‑centred service through accurate documentation and completion of essential administrative tasks.
