The Team The Discharge Planning Team is committed to delivering the best outcomes to patients across the North‑West region, demonstrating experience, understanding and expertise in the clinical management of patients with complex discharge requirements. The team partners with Nurse Unit Managers, Nurse Manager Patient Flow, Allied Health and Community Liaison Nurse to co‑ordinate discharge processes, ensuring timely access to services and care. The Role As a member of the multidisciplinary team within Nursing Services, the Clinical Nurse Consultant – Discharge Planning provides clinical leadership and direction in the planning, implementation and evaluation of change management processes to ensure efficient and effective discharge processes across Mersey Community Hospital.
- Leading the strategic and operational development and implementation of policies and procedures that maximise patient access.
- Providing day‑to‑day leadership, management and advice to staff regarding the hospital’s discharge process.
- Collaborating with MCH and community health groups to develop partnerships for projects that support best practice in Patient Flow Management. Qualifications
- Works in collaboration and consultation with the Patient Flow Manager, Nurse Unit Managers, Nursing Directors and respective Group Managers.
- Functions as a clinical resource for hospital and community staff on issues and processes related to admission, hospital stays and discharge planning.
- Develops and implements training programs that support efficient and effective discharge planning for nursing, medical and allied health staff, maintaining high standards of continuing education and professional development. Salary and Benefits Fixed‑term, part‑time, day‑worker position, working up to 48 hours per fortnight, commencing from 19 July 2026 until 30 June 2028. Hours are negotiable with the successful applicant. Salary: $118,297 to $125,543 per annum (pro rata). A 12 % superannuation contribution is added. Salary range aligns with the Nurses and Midwives (Tasmania State Service) Agreement 2023. Access to the Grade 4 salary range ($102,439–$109,175) is subject to progression to Grade 4. Post Graduate Allowance of up to 10 % based on qualification type may be available. Salary packaging includes living expenses up to $9,010 and meal entertainment of $2,650 each FBT year, plus other eligible benefits.
- Travel and relocation assistance up to $15,000 for overseas applicants, up to $10,000 for interstate applicants.
