Program Coordinator - Positive Ageing (Elizabeth)

This role is for someone who has a passion for working with older people and brings warmth, energy and confidence to the team. **- Why this role?**

As the Program Coordinator, you will play a key role in coordinating and delivering community programs that support older people, with a particular focus on our Men’s Programs. You will assist clients in creating individual goals, building capacity and fostering connections. You will help create a safe and inclusive environment, where people feel comfortable to connect, participate and belong.

You will have a sound understanding of Aged Care and ensure that program delivery is professional and in line with the Commonwealth Home Support Program (CHSP) guidelines and in accordance with the Aged Care Act (2024).

This role suits someone who can balance warmth with structure, build solid qualified rapport while maintaining appropriate boundaries, and bring a thoughtful, person-centred approach to community service delivery. **- What you will get to do**:

- Coordinate and deliver targeted Men’s programs, activities and outings that support wellbeing, connection and individual goals
- Support the delivery of groupbased programs, such as exercise groups, social groups outings including programs that specifically support men
- Build and maintain strong, professional relationships with clients, staff, volunteers, contractors and community partners
- Support clients through intake, review and exit processes in line with CHSP requirements
- Gather feedback, evaluate outcomes and contribute to continuous improvement
- Coordinate and support volunteers and contractors involved in program delivery
- Ensure programs are delivered in line with CHSP requirements, Aged Care legislation, Strengthened Quality Standards and Council policies
- Accurately record client and program information in My Aged Care and Council systems
- Work collaboratively with the Positive Ageing team to deliver highquality, compliant community services

**What you will bring**:

- Demonstrated experience delivering community, aged care or Men’s programs in a professional, personcentred environment
- Experience building meaningful, professional relationships with people from diverse backgrounds, including older men participating in groupbased programs
- An understanding of (or willingness to build knowledge in) aged care frameworks, CHSP, wellness and reablement principles
- Excellent interpersonal and communication skills, with the ability to balance empathy, professionalism and clear boundaries
- Strong organisational skills, initiative and the ability to manage competing priorities
- Confidence coordinating volunteers and working collaboratively with internal and external stakeholders
- Qualifications or relevant experience in Aged Care, Community Services or a related field
