Northern Health are seeking applications from motivated General Medical Practitioners to increase their Chronic Kidney Disease knowledge, management capabilities and connection to the Northern Health Nephrology team by participating in a weekly Nephrology Outpatient Clinic for a fixed term position of 12 months. Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north. Services are provided through our six main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg. About the Role: This role is a fixed term arrangement for 12 months that requiring working in a weekly Wednesday Nephrology outpatient clinic (1 session, 3.5 hours/week) at Northern Health's main Epping campus, managing patients with established renal disease. This role is designed to enrich the knowledge, familiarity and capabilities in managing renal disease of the General Practitioner in their community practice. This role is responsible for assessment and management of patients attending a Nephrology Outpatient Clinic. Patients attending this clinic will be of varying stages of chronic kidney disease including:
- Early stage kidney disease
- Late stage chronic kidney disease encompassing end stage kidney disease planning involving:
- Renal transplantation assessment
- Dialysis planning
- End Stage Kidney Disease supportive care
- Established End Stage Kidney Disease including:
- Dialysis care
- both haemodialysis and peritoneal dialysis
- Renal transplant care Within the spectrum of chronic kidney disease, the General Medical Practitioner will gain exposure to numerous renal diseases and their management strategies including:
- Diabetic Kidney Disease
- Proteinuric Kidney Diseases
- Glomerular disease (Glomerulonephritis)
- Genetic/Inherited Kidney Disease (in particular Polycystic Kidney Disease) To be successful in this role you will have:
- Desire for enhancing knowledge and management/shared care for patients experiencing Chronic Kidney Disease in the local community.
- An ability to promote and disseminate learnings from their own experiences to fellow General Practitioners to enhance the care for many patients experiencing Chronic Kidney Disease in the community.
