Disability Support Worker Full Time 40 Hr /week Moonee Ponds

Providing personal care support to promote optimum health and well being; Providing direct support to assist participants to access appointments, social and leisure pursuits; Working closely with families, professionals and other stakeholders. Working to meet the unique needs of each individual by following their individual Support plans and working towards their bigger life goals, assisting them to capture their achievements along the way; Effective documentations to ensure high quality, participant focus, secure and well organized operational services. Assisting with Medication and meal preparation.

Qualification and Experience

Certificate 3 (or above) in Disability or Induvial support

VIC Drivers Licence & Own Reliable Car First Aid and CPR

NDIS Screening Check

Working with Children's Check Covid Vax
- 2 dose minimum Valuable written and verbal communication skills

Manual Handling Certificate is a Must

Working Hours:

Shift 1: 9am
- 4pm

Shift 2: 4pm
