- Permanent Part Time 32 hours per fortnight About the role As the Grade 2 Social Worker within the Motor Neuron Disease (MND) Clinic, you will provide specialist Social Work assessment, intervention and ongoing support to clients living with progressive neurological diseases, particularly MND. Working collaboratively with the MND Clinic Coordinator and multidisciplinary team, you will support clients and families to navigate complex care needs across hospital, community and disability services. The role focuses on delivering evidence-based, person-centred care while promoting continuity of care and improved quality of life for clients across the Barwon South West region. You will also contribute to service development, quality improvement initiatives, professional leadership and the supervision and support of students and less experienced staff. Key responsibilities
- Deliver comprehensive Social Work assessment, care planning and ongoing support for clients and families living with MND
- Coordinate referrals and collaborate with internal and external providers to ensure continuity of care
- Contribute to service development, quality improvement and evidence-based clinical practice initiatives
- Provide education, supervision and professional support to students and multidisciplinary team members What we’re looking for
- AASW accredited Social Work qualification with eligibility for current AASW membership
- Demonstrated experience working with people living with progressive neurological diseases, particularly MND
- Strong communication, problem-solving and multidisciplinary collaboration skills
