About The Role Join our Intensive Therapeutic Care (ITC) team as a Case Manager to provide tailored, high quality and effective case management to vulnerable children and young people in our program. This is a permanent, full‑time position based at West Gosford, with some travel across Central Coast. This role sits within our ITC and Therapeutic Supported Independent Living (TSIL) program, which provides an alternative to long term residential care, focusing on recovery from trauma, the achievement of positive life outcomes and a broad range of placement options for children and young people. CatholicCare provides flexible working options and various learning & development opportunities. Flexibility to work some additional hours, including at weekends, and to undertake on‑call duties is required for this role from time to time. Reporting to the Area Manager, you will be supported by an experienced team of passionate people working together to change the lives of young people and promote change in the Community. What You’ll Do
- Undertake comprehensive case planning and holistic assessments to promote the safety, wellbeing, and permanency of children and young people in care.
- Work collaboratively with young people and their families to develop clear, strengths‑based pathways that support transitions through care and ultimately exit from the care system.
- Maintain accurate and timely documentation, including case notes, panel reports, care plans, and other statutory and organisational casework reports.
- Build and sustain positive, professional relationships with young people, families, carers, and key internal and external stakeholders to support coordinated service delivery.
- Facilitate permanency planning, including preparing and submitting referrals for Leaving Care, and TSIL/SIL supports to ensure continuity of care and successful life transitions. What You’ll Bring
- Degree qualifications in Social Work, Psychology or equivalent and ideally additional training or qualifications in developmental trauma, child protection, client intervention and case management.
- Demonstrated case work experience working with children and young people who have experienced complex trauma.
- Experience working with families who have been in contact with the child protection system.
- Solid understanding of the Intensive Therapeutic Care model.
