We are seeking passionate and dedicated professionals to join the **Youth Enhanced Services (YES)** team, a program funded through local Primary Health Networks (PHN). YES is designed to support young people aged 12
- 25 with complex and longstanding mental health issues, along with their families. This program targets the "missing middle"—young people who are too complex for primary mental health care but not acute enough for tertiary services, often experiencing inconsistent or inadequate care.
The **BounceBack **program has been delivering high-quality, wrap-around care since 2019 in the Casey, Cardinia, Dandenong, and Kingston regions. The program offers comprehensive support through an interdisciplinary team of clinicians, including mental health clinicians, psychiatrists, family therapists, peer support workers, and carer peer support workers.

As part of our integrated team, your key responsibilities will include:
- Completing comprehensive screening and assessment and providing case coordination and treatment to young people who present with complex and chronic mental health issues
- Formulating recovery plans, as well as assessing for and completing appropriate referrals
- Collaborate effectively with other care agencies, interagency networks, and educators to create a supportive network
- Actively monitoring young people, including follow ups as required to meet your clients’ goals

**Qualifications**: If you’re an experienced and motivated Mental Health Clinician who is passionate about supporting young people and their families within our community, then we want to hear from you!
Essential qualifications and experience include:
- Undergraduate qualifications in Occupational Therapy, Psychology, Nursing, Social work, or related discipline
- Eligibility for membership in qualified field
- Demonstrated experience in assessment, case-management and evidence-based psychological interventions to young people in the context of severe mental health issues
- Experience working as part of a multidisciplinary team, and alongside young people and families for a minimum of 4+ years in the context of severe mental health.
- Completion of a Criminal History Check and Employee Working with Children Check (or State equivalent) prior to commencement of employment and as required by legislation and policy during employment, as well as a duty to disclose relevant information that may arise after employment has commenced
- Current state-based driver’s license
