The Team The Community Transition Care Program offers short‑term, goal‑oriented support for older people transitioning from acute or subacute hospital settings to the home environment. We are a dynamic multi‑disciplinary team supporting clients to optimise functioning and independence. The Role The TCP Clinical Nurse works closely with the TCP Manager to triage referrals and assess clients’ current care needs. Through liaison with referring teams, the Clinical Nurse ensures that risks and barriers to safe discharge home are addressed, and that appropriate supports are arranged. The role attends a comprehensive initial home visit assessment to identify the needs of older persons and their carers. Taking a person‑centred and multidisciplinary approach, the role provides clinical expertise, support, and advice to the multidisciplinary team in the management of clients with complex nursing care needs, including risk screening and assessment of falls, pressure injury, wound management, cognitive impairment, medications, continence, and pain. The TCP Clinical Nurse may provide short‑term direct clinical nursing interventions or facilitate onward referrals to support the ongoing nursing needs of clients. As required, the Clinical Nurse liaises with clients’ external healthcare services, such as General Practitioners, Specialists, and Service Providers, to ensure a consistent and multidisciplinary approach to care. You will need
- Highly developed clinical nursing knowledge, skills, and judgement with experience providing evidence‑based primary health care to older people with age‑related needs or cognitive impairment.
- Ability to work in a leadership partnership with the Nurse Manager to maintain a strong professional environment by assisting with planning and coordination of staff and resources.
- Demonstrated ability to work autonomously and as part of a multidisciplinary team to provide effective and efficient client care and discharge planning within established operational frameworks and using a range of information technologies. Details of appointment Permanent, part‑time, day worker position, working 68.4 hours per fortnight, commencing as soon as possible. Hours to be negotiated with the successful applicant. Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full‑time, part‑time, and casual vacancies. Salary $109,767 to $115,813 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount. Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023. Additional allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10% based on qualification type. Salary Packaging You’ll have access to salary package advantages including living expenses up to $9,010 and $2,650 in meal entertainment each FBT year, plus other benefits if eligible. Benefits available to eligible candidates
- Travel and relocation assistance support available up to $10,000 for appointees from interstate.
