About Your Role

Scope has an exciting and unique prospect for a Service Transition Coordinator to work within our Intake Division.

Work with a team-oriented team to advance client, family and employee experiences

Identify, explore and advocate for opportunities to improve on how we onboard clients

Develop and maintain respectful, productive and ethical relationships with relevant internal and external stakeholders

Maintain accurate documentation and communicate with stakeholders to clarify expectations

What We Look For

Ability to collaborate with internal and external stakeholders

Priortise and manage competing deadlines

Experience within disability services and/ or related sector
