We’re looking for a Part Time Social Support Group Assessment Officer to provide timely and accurate assessment, care planning and review of eligible members of the community who wish to participate in our Social Support Group.
- Deliver high-quality assessment and care planning to support social connection
- Assist community members in accessing programs under CHSP and HACC-PYP
- Support Social Support Group assistants and volunteers in program delivery
- Work collaboratively with the team under the guidance of the Social Support Group co-ordinator / Community Health Services Manager
- Maintain accurate client records and support continuous improvement of services
- Not-for-profit salary packaging up to **$15,900**per year + **$2650** Meal and entertainment card

**About You**:

- A Cert 3 in Aged Care, Disability, Home and Community Care, Lifestyle or equivalent experience
- Experience in assessments and goal-directed care planning
- Solid communication, organisation and time management skills
- Understanding of Active Service Model principles and wellness frameworks
- Proficiency in computer systems and client documentation
- Current Victorian Driver’s Licence
- Valid Police Check, First Aid and CPR Certificate
- Working with Children Check
- A genuine commitment to inclusive and person-centred care
