Description
Experienced AHPRAH registered (unrestricted) nurse practitioner required to work with the Street Doctor service in Armadale, with possible availbilty to cover other scheduled clincis. Armadale clinics are currently scheduled for 9am-1pm Tuesdays, however this may be able to be negotiated if required. Must have Primary Care experience with knowledge of MBS billing Best Practice Software experience preferred but not essential Must be able to work autonomously with support of the Registered Nurse during consults- No onsite GP supervision available 360 Health + Community is a person-centred health service providing care and support to people when, where and how they need it most. Our teams deliver well known and trusted programs across Western Australia to help body and mind across the lifespan including Street Doctor, ALIVE, headspace, NDIS Support Coordination and Therapy, Commonwealth Home Support Program and a variety of other allied health and support services.

Our GP Services are all about improving the health & wellbeing of homeless, transient, and disadvantaged people in Perth. We currently run 11 Street Doctor clinics each week across 7 locations and provide medical services within the Karlup Service at the Ruah Centre for Women and Children as well as at other refuges and shelters.

We are looking for a casual Primary Care Nurse Practitioner for placement in our Armadale location, with flexibility to cover other Street Doctor Clinics as needed.

Job Summary The role of the NP includes evaluating, diagnosing, and treating the client along with delivering education and counselling help to reach their individual health goals. The role within the Street Doctor team requires compassion and empathy for vulnerable groups targeted by the service, with understanding of challenges faced by the client cohort experiencing homelessness and social disadvantage. The NP will assess and diagnose, using physical assessment, ordering diagnostic tests, then initiating treatment through non-pharmacological or pharmacologic measures, or consults with other providers or specialties to ensure the patient receives the necessary care. The NP plays an important role within the multidisciplinary team collaborating with support services as needed to meet the patient’s healthcare needs.

KEY DUTIES & RESPONSIBILITIES

· Provides advanced clinical assessment/reasoning skills and autonomous care to patients in accordance with Nurse Practitioner legislation and within individual scope of practice.
· Provides collaborative clinical and professional leadership and assists in the maintenance of standards of nursing practice and patient care.
· Actively promotes cooperative relationships with patients, peers and all members of the multidisciplinary health care team.
· Coordinating patient services through planning and management of care, and by providing information and feedback between the services, patients and GP.
· Improving health outcomes in vulnerable population groups by contributing to and enhancing the management and prevention of ill health through health screening, immunisation, recall, patient education, outreach services, acute and chronic condition management.
· Flexibility to work with clients in a non-traditional setting for optimal engagement.

REQUIREMENTS & ATTRIBUTES

Essential
· AHPRA registration as a Registered Nurse with endorsement as a Nurse Practitioner.
· Advanced clinical experience in the assessment and treatment of illness and injury and health promotion relevant to the role and within individual scope of practice.
· Demonstrated extensive professional and/or leadership experience.
· Demonstrated clinical problem-solving skills including conceptual and analytical ability.
· Advanced interpersonal, communication and negotiation skills (written and verbal).
· Knowledge and understanding of MBS billing items and related regulations.
· Knowledge, understanding and experience in quality improvement, its practical application in meeting patient’s needs and quality service delivery.
· Knowledge, understanding and experience of research findings to support evidence-based practice.
· Experience in quality improvement, its practical application in meeting patients’ needs and its relationship to strategic development.
· Sound knowledge of relevant legislation and health regulating acts and legislative obligations.
· Computer literacy.
· Evidence of appropriate and continuing professional indemnity insurance.
· National Police Clearance.
· Vaccinations in line with relevant government directives.
· Current ‘C’ class drivers licence (MR/HR license desirable).

Desirable
· Demonstrated understanding of key cultural issues influencing practice, patient relationship and patient need.
· Experience working with vulnerable populations.
· Experience within a general practice setting.
· Experience with the accreditation process and quality improvement requirements.
· An understanding or experience of the RACGP Standards for general practices.

The hourly rate is $75
