GSHS offers a broad range of services that meet the needs of a diverse and sparsely populated area. With two main acute facilities based at Leongatha and Korumburra, GSHS offers a broad range of specialist, general, acute, subacute, ambulatory, residential aged care and community services.**About the Role** GSHS is seeking a suitably qualified &/or experienced Social Worker to join our team in a **casual** capactiy.- With a diverse and varied caseload, working with inpatients, outpatients and community counselling services, the position provides the prospect to work with people to provide a truly holistic service, supporting people through the full care cycle.- The position covers a catchment large enough to provide good support networks and clinical variety but small enough to still provide personalised care.**About You** Reporting to the Community Services Manager, via Social Work Team Leader, the successful applicant will be responsible for:
- Providing quality support to clients by completing assessments, service delivery (including counselling) and discharge planning tasks consistent with best practice standard.
- Participating in a coordinated team approach to patient care.
- Participating in the education of patients, staff, community and students.
- Staff have access to supportive staff mentoring, education and development programs. Salary Packaging is available for all staff.
