About the Social Work Department The Social Work department is committed to promoting the highest level of care for our patients. In realising this goal, the department leads and excels in person‑centred care, evidence‑based practice, innovation and quality improvement. The department provides Social Work services on all three campuses of Austin Health, from the Emergency Department through to bed‑based services and ongoing ambulatory care. About the role As a Grade 1 Social Worker you will be responsible for providing clinical social work services to inpatients across one or more of our Austin Health sites. A regular day will see you work clinically with patients, attend family meetings and non‑clinical meetings, and undertake other duties that support the Social Work department and its operations (as appropriate for Grade 1). Responsibilities
- Apply your Social Work knowledge and skills to assess, treat and plan the discharge of patients.
- Undertake comprehensive psychosocial assessments, and develop and implement appropriate intervention plans, including care planning, counselling, and discharge planning for patients and their families.
- Participate in quality improvement, service improvement and/or research. Qualifications
- A degree in Social Work.
- The ability to work and contribute to team culture and work within a multidisciplinary setting.
- Solid communication skills, both written and verbal. Benefits
