**NDIS Support Worker
- Community Access (Minto NSW 2566)**

NITPLANS Care Services is seeking compassionate and reliable **NDIS Support Workers** based in or around **Minto NSW (2566)** to support an adult participant living with disability. This role focuses primarily on **community access and social participation**, supporting the participant to engage meaningfully in the community.

**About the Participant**
- Adult male participant
- Primary disability: Stroke
- May experience mobility and/or functional limitations
- Requires support to access the community and participate in activities
- Motivated to increase independence and social engagement

**Role Details**
- **Location**: Minto NSW 2566
- **Shifts**: 2-3 days per week (adaptable)
- **Hours**: Approximately 4-6 hours per shift
- **Type**: In-person supports
- **Start**: Immediate start preferred

**Key Responsibilities**
- Support with **community access and social participation**:

- Assist with **transport to appointments, outings, and activities**:

- Encourage and support **independence and confidence in the community**:

- Assist with **light daily living tasks if required**:

- Support engagement with **allied health and community services**:

- Provide **person-centred support** aligned with participant goals

**Essential Requirements**
- Certificate III or IV in Individual Support (or equivalent)
- Current **NDIS Worker Screening Check**:

- **Working With Children Check (WWCC)**:

- Current **First Aid & CPR**:

- Valid **Driver’s Licence** and reliable vehicle
- **Comprehensive car insurance**:

- 100 points of ID
- Experience supporting participants with **neurological conditions (e.g. stroke)** highly desirable
