Warramunda Community Care Case Manager Full Time
- Fixed Term Contract Are you passionate about supporting older people to live independently and safely in their own homes? Warramunda Community Care is seeking a compassionate, organised and motivated Case Manager to join our growing team. This is an exciting opportunity to work within the evolving Support at Home program while making a genuine difference in the lives of our consumers and their families. About the Role As a Case Manager, you will coordinate non-clinical support services for consumers receiving services under the Support at Home program, private-funded services, and future service pathways. You will work closely with consumers, families, carers and service providers to ensure consumers receive high-quality, person-centred support that enables them to remain living independently at home for as long as possible. This role combines care coordination, relationship management, community engagement and service growth. Key Responsibilities
- Conduct non-clinical assessments and develop person-centred care plans
- Coordinate and monitor services for consumers receiving Support at Home and private-funded services
- Monitor consumer budgets and expenditure
- Liaise with consumers, families, carers and external providers
- Refer all clinical concerns to the EEN Clinical Care Partner or appropriate clinical providers
- Respond to current enquiries and support consumer intake processes
- Build strong relationships with community organisations and referral partners
- Represent Warramunda Community Care at expos, community events and networking opportunities
- Assist with audits, compliance tasks and quality improvement activities
- Maintain accurate documentation and consumer records About You You are someone who genuinely cares about people and enjoys building strong relationships within the community. You are organised, proactive and able to manage competing priorities in a fast-paced environment. You will also bring

- Certificate IV in Aged Care or similar qualification
- EEN qualification highly regarded
- Current First Aid & CPR
- Current Driver’s Licence
- NDIS Worker Screening Check
- Current Police Check
