Disability Support Worker – Mobile Rapid Response Team CareChoice is expanding our Mobile Rapid Response Team and seeking experienced Disability Support Workers (we want the best of the best!) to deliver urgent, short‑term support to clients across Metro Melbourne and Regional Victoria. If you enjoy variety, autonomy, problem‑solving, and stepping into quick‑paced environments, this is the role for you. About the Role In this dynamic position, you will provide interim support (up to 3 months) to clients during critical transition periods such as hospital discharge or provider changeovers.
- Metro and regional assignments
- Travel, meal allowances and accommodation provided where required
- Occasional overnight stays You’ll work closely with our Intake and Operations teams to stabilise new supports and deliver safe, person‑centred care from day one. What You’ll Be Doing
- Deliver high‑quality, person‑centred, and trauma‑informed support
- Respond to urgent or complex situations at short notice
- Assist with new client intakes and transitional care
- Provide personal care, community access, and daily living support
- Complete complex care tasks and high intensity support (as trained/competent)
- Maintain accurate documentation and communicate changes in client needs What We’re Looking For
- Certificate IV in Disability, Aged Care, Community Services or similar
- Experience supporting clients with complex physical and/or behavioural supports and high intensity support
- Current First Aid, CPR, Victorian Working With Children Check (Employee), and NDIS Worker Screening Check
- Victorian driver’s licence and a reliable, insured vehicle
- Strong communication and problem‑solving skills
