As a Care Consultant with BaptistCare, you won’t be pushing people through a system. You’ll be walking alongside them — helping older Australians stay safely and independently in the homes and communities they love, in a region you know.
WHAT WE OFFER
$44.65 per hour plus NFP salary packaging up to $18,550 per year means more take home pay without a pay rise!

THE ROLE
You’ll be the primary point of contact for a caseload of Home Care Package customers across the North
- West Tasmania assessing their needs, coordinating their services, managing their budgets, and visiting them regularly to make sure their care is genuinely working for them.
Conduct intake, initial, and ongoing assessments of aged care-related needs
Develop and maintain goal-directed care plans within each customer’s home care budget
Coordinate and facilitate supports and services across the Home Care Packages program
Manage package spend and advocate for customers’ needs and preferences
Conduct regular home visits and ongoing monitoring across the Bendigo region
Build strong relationships with customers, families, carers, and community stakeholders
ABOUT YOU
You bring clinical or case management foundations, genuine empathy, and the ability to work independently across a regional caseload.
Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
Prior case management experience, particularly in home care or community services (highly regarded)
Experience completing initial and annual customer assessments and care planning
Knowledge of the Home Care Packages program and the broader home care sector
Division 1 Nursing registration (desirable but not essential)
Based in Tasmania with a current driver’s licence and willingness to travel across the area
National Police Check (or willingness to obtain)
Proof of working rights in Australia
