## Clinical AssessorApplylocations: Morayfield, Queensland: Nambour, Queenslandtime type: Full timeposted on: Posted Yesterdayjob requisition id: JR **Join an Organisation dedicated to Closing the Gap in Indigenous Health****About Us?**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.The focus of IUIH Aged Care team is to work with the local indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services and eligible aged care clients.**About the Role?**As Aged Care Clinical Assessor, your role plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services to older Aboriginal and Torres Strait Islander people, operating in one or both of the Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). The role is responsible for conducting culturally appropriate Comprehensive Assessments and occasionally Home Support Assessments for eligible older Aboriginal and Torres Strait Islander individuals, in accordance with the policies and procedures of the Aged Care Single Assessment System.Your day-to-day duties will include:* Undertake Comprehensive Assessments that require clinical judgement of clients allocated by the Team Leader, with their consent and applying the IAT User Guide, in compliance with the procedures and processes detailed in the My Aged Care Assessment Manual and IUIH’s Standard Operating Procedures Pilot.* Refer clients to appropriate services, assess complexity and vulnerability, and assist with linking support, reablement, and short-term case management.* Undertake Home Support Assessments from time-to-time as allocated by the Team Leader Home.* Support Assessments, in compliance with the procedures and processes detailed in the My Aged Care Assessment Manual and IUIH’s Standard Operating Procedures for the Pilot* Complete Support Plan Reviews referred from the Team Leader.* Initiate Support Plan Reviews, without requiring a request or scheduled SPR.* Attend planning, program, and team meetings as required.* Provide periodic performance and evaluation reports to the Team Leader, ensuring accountability and ongoing improvement in service delivery**About You?*** Tertiary qualification in a health-related discipline directly related to health, aged care or related specialist area.
