- Part Time Position No: 00115402 Closing Date: 2026-06-08 4:00 PM CRVCID731815 Rehabilitation in the Home (RITH) provides short to medium term hospital substitution allied health therapy for patients at home. The service aims to facilitate early supported discharge from hospitals or avoidance of hospital admission for patients. RITH has offices based at the following locations:
- North: Joondalup, Sir Charles Gairdner Hospital
- East: Armadale, Royal Perth Hospital, South Guildford
- South: Fremantle, Rockingham, Mandurah RITH are currently seeking passionate Senior Social Workers to join our team in the following capacity
- North RITH
- 1 FTE Permanent position . Based at the Joondalup RITH office with leave cover at SCGH. Immediate start.
- South RITH
- 0.6-0.7 FTE Permanent position 0.2-0.3 FTE based at Fremantle and 0.4 based at one of the three south RITH bases. FTE may be consolidated into one position or split dependent on applicants’ preferences and suitability. Position will commence in latter half of 2026. Hear what our patients say about our service: About the Role The successful applicant/s will be a part of a multidisciplinary team, providing Social Work input to patients in the clinical specialty area of RITH using advanced Social Work clinical practice skills. This Social Work role plans, implements, coordinates and evaluates delivery of Social Work services with a focus on patient safety and quality of care. Practices as a Senior Social Worker and ensures practice is in accordance to the Australian Association of Social Workers Practice Standards 2023 and Code of Ethics 2020 and/or the Society of Professional Social Workers Code of Ethics and SMHS policies and guidelines.
