About The Role We are looking for a passionate AHPRA registered professional (Nurse, Occupational Therapist or Social Worker) to join our quick-growing team in Western Australia. In this role, you’ll manage relationships and oversee service compliance for a portfolio of clients who have chosen The CareSide to fully manage their home care services. Your Responsibilities
- Complete client assessments and care plans
- Mitigate client challenges and facilitate a smooth home care experience
- Supervise care delivery
- Review client progress notes and ensure corrective actions are implemented
- Meet with prospective clients and explain the value proposition of joining The CareSide
- Report and manage incidents and hazards
- Develop relationships with clients and their families and proactively seek feedback to ensure we’re meeting their service expectations Your Background We are looking for a compassionate, reliable and organised individual who will thrive in a collaborative environment while actively assisting aged care clients and providing professional advice.
