**The Role**
As a Support Coordinator / Social Worker, you will play a pivotal role in providing exceptional support services to Level 2 and 3 participants requiring support coordination.
Your primary responsibilities will include:
- Supporting participants to understand their NDIS plans and exercise choice and control.
- Connecting participants with supports, negotiating service agreements, and removing barriers.
- Assisting with plan reviews, reports, and achieving measurable outcomes.
- Building trust and maintaining continuity when stepping into existing caseloads.
- Collaborating with stakeholders to strengthen participant networks and resilience.
- Upholding Supporting Your Way of Life's values and person-centred practices.
**What We’re Looking For**
- Bachelor in Social work essential, Australian Association of Social Workers (AASW) membership with 2 years' experience as a Social Worker or Support Coordinator.
- Needs drivers license, Police Check and Working with Children's Check.
- Looking for a permanent Social Worker / Support Coordinator with neurodivergence and mental health experience.
- Role is a varied position with active support work, stakeholder engagement, intake assessment, crisis intervention and carer and family support
- Strong knowledge of the NDIS and disability support sector.
- Excellent communication, organisational and problem-solving skills.
- Ability to work independently while staying connected to a team.
- A genuine passion for making a difference in the lives of people with disability.
