Support Coordinator ( Service Navigator) Part time or Casual offered
**Company Overview**: Community Care Links Services is a NDIS registered provider operating within Gold Coast.
Seeking an experienced Support Coordinator ( level 2/3 ) to join the Team, you need to have 1**2 months minimum experience**
(A sub-contractor seeking to work for a registered provider will also be considered)
**Benefits**:
- WFH but have an office day for peer support/supervision.
- Immediate start or start after Xmas

- Access to already established systems to support your role
- Internal peer support, training & supervision so you never feel alone
**Position Overview & Key responsibilities**: Your role will involve the following:
- Working with NDIS participants and a diverse age range.
- You will be building solid relationships with clients, their families & service providers.
- Your focus will be to build clients capacity to understand their funded supports, & how they utilise their NDIS plan, as well as their support plans and staying on top of their budget expenditure.
- You will be supporting clients to negotiate service agreements to ensure supports meet their goals.
- You will be writing reports whilst adhering to NDIS requirements and supporting participants through scheduled and unscheduled review meetings/processes.
- Your role will entail working closely with a number of Service providers, LAC's and the NDIS.
- You will be making a big difference to participants with a range of conditions, including neurological conditions, ASD, psychosocial and many more.
- Document client records including case notes, service agreements and reports
**About you & essential requirements** As a key member of the team, we consider it essential you:
- Experience essential as you will be taking on an existing case load
- Preferably you hold a bachelor’s Degree in community related field but minimum Diploma will be considered.
- Previous Support Coordination experience or Case management experience.
- Ability to multitask as this is a busy role
- Ability to manage Complex situations
- An ability to work both independently
- Confident oral and written communication skills.
- Blue Card/WWCC
- Driver's license and vehicle
- First Aid Training
- Experience in the community _(desirable)_
- NDIS Worker Screening Check (_desirable)_
