In this role, you will coordinate and manage client-centred care that is safe, timely, and effective, while supporting vulnerable individuals through personalised service plans. Responsibilities include handling complex cases, developing and updating care plans, facilitating clinical support, overseeing client budgets, and collaborating closely with internal teams such as rostering, compliance, and HR. You’ll also promote Home Care Packages, lead meetings, manage incidents or complaints, and ensure high standards of service through staff support and training. A strong understanding of aged care funding (HCP, CHSP, DVA) and a commitment to culturally appropriate care are essential.
