Community Registered Nurse (Perth)

About the role The Community Registered Nurse supports older clients to remain safe, independent and well in their own homes. Working across areas south of the river, you’ll deliver holistic clinical care, complete assessments, support chronic disease and wound management, and provide education and guidance to support workers and carers. You’ll work closely with internal teams and external health professionals to ensure clients receive high quality, evidence‑based care that promotes wellness and enablement. This is a permanent part-time position working up to 60 hours per fortnight. Due to client needs, applicants must have availability on Mondays and Fridays and be available on weekends as required, and the ability to travel throughout the south metropolitan region. About you You’re a compassionate and adaptable Registered Nurse who enjoys working independently while remaining connected to a collaborative team. You have solid clinical judgement, excellent communication skills, and a genuine commitment to supporting older people within the community. In addition, you will also possess:
- Current AHPRA registration as a Registered Nurse
- Experience working within community, aged care or complex health environments
- Knowledge of person‑centred care, clinical assessment and care planning
- Confidence supporting clients with chronic conditions, dementia and complex care needs
- Strong organisational and time management skills
- Sound computer skills including Microsoft Office applications
- Current Driver’s Licence and willingness to travel throughout Perth south of the river
