- $120,680.00 per annum + 12% Superannuation + Salary Packaging Location: Braeside Hospital Applications close: 07/06/2026 Position Overview SWSLHD is seeking an experienced physiotherapist to join the community-based Liverpool Transitional Aged Care Program (TACP) as a full-time permanent level 3 physiotherapist. The role is based within a multidisciplinary team at the Liverpool TACP site and focuses on the reablement of older adults living in the community. Responsibilities
- Assess TACP clients in their homes, setting holistic goals, developing client‑centred treatment plans and discharge planning.
- Commit to evidence‑based practice and lead quality improvement initiatives within the physiotherapy team.
- Provide ongoing education, training and supervision for level 1/2 physiotherapists, allied health assistants and students.
- Participate in guideline and policy review, staff education, and district‑wide quality improvement/EBP projects. Qualifications
- Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience working in aged care and/or rehabilitation.
- Ability to work independently and collaboratively within a multidisciplinary team.
