**About the Role**: Bayside Care and Support is seeking a motivated officer to join the fast-paced Service Intake and Commencement team to assist the onboarding of recent aged care clients.
You will be responsible for:
- Managing the My Aged Care Portal including accepting appropriate referrals on a rotational basis.
- Actioning incoming referrals for services received from assessment services including completion of service priority of access, an agreed service plan including consideration of safety issues and confirmation of service fee/s.
- Prioritising referrals based on client needs and completing an agreed number of client referrals per week.
**Key Selection Criteria**: To be successful in this role, you will bring the following skills and competencies:
- Demonstrated experience working with older people and people with a disability in a community setting.
- Knowledge and understanding of community based aged care services and programs essential.
- Well-developed communication skills enabling client needs and preferences to be effectively documented for Support Workers who deliver services.
- Tertiary qualifications in a health, allied health, social science or related discipline desirable.
**Mandatory pre-employment requirements**:
- Be willing to undergo and complete a satisfactory criminal history check.
- Provide or obtain an employment-based Working with Children check
- Be fully vaccinated with at least two approved COVID-19 vaccines or have proof of medical exemption, in accordance with Bayside Care and Support's COVID-19 requirements.
