**About the role**

As a Health and Wellness Promoter you will work under our tested multidisciplinary model of care that has significant proven benefits for getting our clients onto their best health pathway. Our model uses a comprehensive assessment battery and case conferencing process to inform targeted prescription, with the client journey culminating in ongoing exercise and service participation.

This role will be based from The Philip Kennedy Health and Fitness Centre and on-road working across various funding streams and programs including: Commonwealth Home Support Program, Support at Home, Community Transitional Care and Restorative Care Programs.

Key Responsibilities include but are not limited to;
- delivery of group based and 1:1 exercise programs under the guidance of an Exercise Physiologist
- contributing positively to multi-disciplinary care planning for our clients
- demonstrating experience in person-centred practice, service continuous improvement, evidence based treatment and service delivery
- proactively delivering evidence-based reablement pathways that ensure client physical, social and mental health benefits
- demonstrating willingness to work as part of a team and to increase your personal skill set

**About you**

We are looking for people who will be the difference in the lives of our residents and clients.
- Certificate IV or higher in Fitness, Exercise Science, Personal Training, Exercise Physiology or equivalent including current accreditation with Physical Activity Australia, AusActive or Exercise & Sports Science Australia
- full and current unrestricted Australian Drivers licence
- demonstrated delivery of progressive resistance training programs with older people
- sound understanding of healthy ageing and that physical function underpins the ability to remain engaged meaningfully in personally valued pursuits
- ability to efficiently analyse information, respond to and advocate on behalf of client needs
- ability to identify best practice fitness program delivery and report to management
- exceptional interpersonal skills as well as the ability to maintain positive working relationships with external stakeholders
- strong organisational and time management skills
- proficient computer and keyboard skills and the ability to use Google Suite (e.g. Docs, Sheets, Gmail etc.) and various in-house databases
- applicants with a Bachelor of Exercise Science will be highly regarded
