- Support NDIS and aged‑care participants in community, clinic and home settings across a diverse caseload.
- Develop long‑term relationships, track participant progress, and coordinate care with multidisciplinary teams.
- Receive dedicated 1:1 supervision, monthly group supervision and support from intake & clinical leadership.
- Plan and manage your own diary, with flexibility across community, clinic and work‑from‑home options.
- Located in Campbelltown with remote or hybrid work options; can join the Bankstown team as needed. Qualifications
- Full general AHPRA registration.
- Driver’s licence and own vehicle.
- NDIS Worker Screening, Working With Children Check and Police Check (or willingness to obtain).
- Minimum of 4years post‑graduate experience. Benefits
