We are looking for caring and dedicated **Home Care Worker
- Disability Care / Aged Care** to support aged care and disability clients in their homes.

As a Home Care Worker,
- Help people with disabilities live independent lives by providing care and support.
- Assist with daily activities like bathing, dressing, and meals.
- Organize social and recreational activities to promote inclusion.
- Monitor clients' health and medication and provide emotional support.
- Work with a team and keep records of care.
- Support elderly individuals in maintaining their independence and well-being.
- Help with personal care tasks, such as bathing and grooming.
- Provide companionship and engage in conversation.
- Assist with household chores and organizing activities.
- Monitor health, administer medications, and document care.
- Collaborate with healthcare professionals and family members.

**Qualifications and Requirements**:

- Experience in providing care for aged care or disability clients.
- Knowledge of person-centered care principles and safety regulations.
- Strong communication and interpersonal skills.
- Compassionate, patient, and empathetic attitude.
- Ability to work independently and as part of a team.
- Physical stamina for lifting, bending, and manual tasks.
- Valid driver's license and reliable transportation (if needed).
- Flexibility to work different shifts, including weekends and evenings.
- Preferably with Certificate III or IV (Advantage)
- Preferably with hoist experience (Advantage)
