Job Description As a Community Worker at Serene Country Living Pty Ltd, you will play a crucial role in supporting and empowering our clients to live independently and actively participate in their community. You will work closely with a team of dedicated professionals to provide a range of services to our clients, including personal care, domestic assistance, social support, and transport. Key Responsibilities
- Provide personal care and support to clients in their homes, including assistance with bathing, dressing, and grooming.
- Assist with domestic tasks such as cleaning, cooking, and laundry.
- Support clients to participate in social and recreational activities in the community.
- Provide transportation to appointments, shopping, and other activities as required.
- Monitor and report any changes in clients' physical or mental health to the appropriate team members.
- Maintain accurate and up-to-date documentation of client care and progress.
- Collaborate with other team members to develop and implement care plans for clients.
- Build and maintain positive relationships with clients, their families, and the wider community.
- Attend training and development opportunities to enhance skills and knowledge. Qualifications and Experience
- Relevant diploma or higher.
- Previous experience of minimum of 12 months working in a similar role, preferably in the aged care or disability sector.
- Knowledge of person-centered care and a commitment to promoting independence and dignity for clients.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- First Aid and CPR certification (or willingness to obtain).
- National Police Check (or willingness to obtain). Advantages
- Salary package of $75,000 to $85,000 plus superannuation.
