We’re looking for a passionate Long Term Care (LTC) Worker to join our Out of Home Care (OoHC) team in a fulltime, permanent role, based at our Head office in Ulverstone. This role plays an important part in supporting foster carers and promoting the safety, wellbeing,and stability of children and young people in care. You will work closely with foster families, Child Safety Services, internal teams, and external stakeholders to help achieve positive outcomes for vulnerable children and young people. About the Role As a LTC Worker, you will provide quality, trauma-informed support to foster carers while advocating for the needs of children and young people in care. This is an office-based role within a supportive and collaborative OoHC team, with regular engagement with carers, children and young people, and external agencies. You will have:
- A Diploma of Community Services or similar
- Previous experience in community services, child safety, foster care or a related field
- Excellent written and verbal communication skills
- A current unrestricted (P2 or above) Australian driver's license
- A NDIS endorsed Working with Vulnerable People card or be able to obtain one
- A current Nationally Coordinated Criminal History check or happy to submit to one Responsibilities include:
- Training and assessing potential foster carers
- Conducting annual foster carer reviews
- Supporting carers to build skills and confidence in their role
- Assessing and advocating for the support needs of carers and children
- Maintaining regular contact with foster families to promote placement stability
- Participating in Care Team meetings and collaborative planning
- Building solid working relationships with Child Safety Services and other relevant agencies
- Preparing case notes, reports, and documentation within required timeframes
- Maintaining accurate case management records and financial documentation
