The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth). We are seeking a passionate Community Health Care Registered Nurse to deliver high quality, person centred care to individuals, carers, and families. In this role, you will provide nursing assessment, planning, implementation, and evaluation using evidence based best practice and the principles of Primary Health Care. Working collaboratively with clients, families, GPs, and multidisciplinary teams, you will ensure care is flexible, responsive, and aligned with the Australian Aged Care Quality Standards. You will provide services such as wound management, palliative care, chronic disease management, and continence care across community settings, including homes and clinics. This position offers variety and operates on a 7 day rotating roster. Responsibilities include supervising Enrolled Nurses, coordinating care for complex and high risk clients, advocating for diverse cultural needs, and mentoring staff and students. - Current registration as a Registered Nurse Division 1 with AHPRA with demonstrated experience and clinical skill in general/community nursing as a Registered Nurse including completion of a new graduate Registered Nurse program or equivalent experience working as a Registered Nurse.
- Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services and an ability to liaise with other health professionals to facilitate case management and care planning.
- Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
- Demonstrated effective communication skills (written and verbal) and interpersonal skills.
- Demonstrated commitment to professional development, evidence-based practice and an awareness of changes within the health care setting.
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
- Ability to work a flexible roster, across all days of the week according to the team requirements.
- Current valid NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD. Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
