**About the Role** We are seeking passionate and driven **Community Social Workers** to provide holistic, culturally safe, and person-centred support to individuals and families. In this role, you will work directly with community members, supporting them to access essential services, build independence, and achieve their goals. This is a rewarding opportunity to make a meaningful impact in the lives of people living with disabilities, mental health conditions, and other complex needs.

Conduct assessments to identify the needs, goals, and strengths of individuals, families, and groups to develop personalised support plans.
- Provide practical in-home support, emotional well-being assistance, and promote the development of independence and daily living skills.
- Facilitate access to essential community services, healthcare, education, and social support programs.
- Plan and deliver educational workshops, community engagement programs, and group sessions for children, parents, and young people.
- Monitor client progress, review support plans, and report on changes, achievements, and challenges.
- Refer clients to external service providers, community organisations, or government agencies when additional support is required.
- Act as an advocate for clients, promoting their rights, dignity, and access to essential services while supporting policy change.
- Conduct research on community needs, emerging trends, and best practices to enhance support initiatives and service delivery.
- Liaise with internal teams, community groups, government bodies, and private sector partners to ensure cohesive support for clients.
- Support families and carers through education, relationship-building, and family-centred approaches for children and people with disabilities.
- Maintain accurate client records, case notes, and documentation while adhering to confidentiality and privacy policies.

**Selection Criteria** To be successful in this role, you will need to meet the following requirements: **Essential**
- Qualifications in Social Work, Community Development, Human Services, or a related field (or equivalent experience).
- Demonstrated experience working with Aboriginal, Torres Strait Islander, and diverse communities.
- Current **Working with Children Check (WWCC)** and **NDIS Worker Check**.
- Current **First Aid and CPR Certificate**.
- Valid **Driver’s Licence** and access to a **comprehensively insured vehicle** (required for client transport).
- Physical capacity to assist with manual handling (lifting up to 10kg) and household support tasks.
- High level of empathy, cultural competency, and strong interpersonal communication skills.
- Robust organisational skills and ability to manage time effectively to meet deadlines.

**Desirable**
- Experience supporting people with mental health, disability, and complex care needs.
- Training or certification in **Safe Food Handling**, **Infection Control**, and **NDIS Worker Orientation**.
