**What you'll be doing** Join a dedicated rehab team providing essential social work support to patients and carers in the SMART service. This role blends clinical practice, education, and development.
- Conducting psychosocial assessments and interventions
- Collaborating with medical teams on complex cases
- Engaging in quality improvement and research
- Educating and support staff and social work students
- Participating in supervision and professional development
**How to Apply**
**Essential Criteria**:
- Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)
**Selection Criteria**:
- Demonstrated understanding of psychosocial issues associated with acute and chronic illness and rehabilitation medicine.
- Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex discharge issues.
- Demonstrated clinical experience or understanding of the psychosocial impact of grief, loss and bereavement.
- Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
- Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.
- Demonstrated ability to work with mínimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
- Knowledge of quality assurance and / or clinical practice improvement activities 8. Unrestricted drivers licence and availability to participate in the After-Hours Crisis On Call Social Work Service.
