Community Worker DisabilityTalk Pty Ltd Location: Australia Position: Full-Time Hours: 38 hours per week Salary: $76,515 per annum plus superannuation DisabilityTalk Pty Ltd is seeking a dedicated and compassionate Community Worker to join our growing team on a full-time basis. The successful applicant will work closely with individuals living with disabilities, their families, and support networks to promote independence, community participation, and overall wellbeing. Key Duties and Responsibilities:
- Assess clients' needs and assist in developing appropriate support plans.
- Provide information, guidance, and referrals to community services and support programs.
- Assist clients in accessing disability, health, housing, employment, and social services.
- Advocate on behalf of clients to ensure they receive appropriate support and services.
- Support clients in developing life skills and achieving personal goals.
- Maintain accurate case notes, records, and reports.
- Liaise with families, carers, healthcare professionals, and community organisations.
- Monitor and review client progress and adjust support strategies where required.
- Promote social inclusion and community engagement for people living with disabilities. Essential Requirements:
- Relevant qualification in Community Services, Disability, Social Work, Community Development, or a related field.
- Minimum one year of relevant work experience in community services, disability support, or a related sector.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Current National Police Check.
- Current Working with Children Check (where required).
- Current driver's licence. What We Offer:
