Social Work Clinician Grade 2 – Department of Nephrology Join the Nephrology social work team in a full‑time capacity for parental leave cover to get a taste of our amazing service. - Utilise your specialised social work skills and knowledge to ensure excellence in care for our patients.
- Responsibilities include caring for patients with a new diagnosis, transitioning care from paediatric to adult service, life‑prolonging dialysis, kidney donation, traveling from Tasmania for a kidney transplant, accompanying the team to a regional hospital clinic to assess transplant suitability, and providing end‑of‑life advice.
- Work with a dedicated and experienced social work team within the nephrology department.
- Opportunity for continuity of care across inpatient and outpatient settings. About You
- Qualifications acceptable for membership with the Australian Association of Social Workers.
- Ability to work collaboratively with a large multidisciplinary team.
- Excellent interpersonal and communication skills.
- Skills in comprehensive assessment, discharge planning and counselling.
- Understanding of relevant legislation and practice framework relating to family violence. All appointments are made subject to a satisfactory Criminal History Record Check and satisfactory Working With Children Check.
